The Role: We have an exciting opportunity for a Senior Extra Care Housing Co-ordinator to join the Strategic Commissioning team. The team are responsible for a wide range of commissioned services including Domiciliary Care, Residential and Nursing care, Extra Care, Day Care, Social Inclusion and Support Services. What you'll do: Hampshire County Council commissions care and support services in 21 Extra Care Housing schemes in the county with several more schemes in development. We are looking for an exceptionally competent senior co-ordinator to lead on allocations for new and existing schemes, line manage and supervise 4 Extra Care Co-ordinators (senior case workers) and work with a wide range of partners to promote Extra Care Housing and co-ordinate referrals.
